We will make sure you have the resources and information you need to continue your recovery after discharge. Our Nurse Navigator and other staff will work with each patient and their families to make a solid plan for reclaiming their quality of life.
Discharge marks the transition from hospital to home. To ensure success and continued progress, the Nurse Navigator and other staff will meet with other care providers and the patient to coordinate the necessary resources are available at home to assist with the routines of daily living.
Before discharge you will meet with the Nurse Navigator for a final patient education session to review the previous patient education and the follow-up care plan.
The Nurse Navigator will arrange a follow up appointment with your primary care provider and the surgeon. She will also prepare a report to provide details of your surgery and recovery and other important information including changes in medications, lab results, discharge instructions and consults.
You can expect a follow up phone call from Nebraska Spine Hospital within three days after discharge and at one month post surgery. We will check in to learn more about your post-operative progress, any medical incidents or hospitalizations, physical activity and overall well being.
The phone calls are more than just an opportunity to reconnect, they allow us to track patient experiences and readmissions. This information is used for continuous improvement processes for patient experience and successful patient outcomes.
